This position is designed to facilitate an effective process of the Mercy Care Management Model supporting quality patient care, safety and financial components; promoting integration of a seamless care model; coordinating patient throughput; collaborating with facilities to include leadership coordination as evidenced by metrics, optimizing performance and adoption of best practice.Qualifications: EducationAssociates Degree in Nursing, required (Bachelors Degree in Nursing, preferred)ORBachelors Degree in Social Work, required (Masters Degree in Social Work, preferred) Experience2 years of clinical experience in case management or related field, required CertificationsAmerican Heart Association Basic Life Support (BLS), required Certification in Case Management or pursuing LicenseRN (Registered Nurse) in the state of practice is required for RN SupervisorLCSW or LMSW in the state of practice is preferred for SW Supervisor OtherDemonstrated leadership skills and excellent clinical competence, including proficiency in nursing improvement processes. Demonstrated excellent interpersonal team-building, collaboration, negotiation, problem-solving, as well as spoken and written communication skills. Must be able to safely and successfully perform job-related functions, with or without reasonable accommodation required by federal, state, or local law.
