Description: POSITION SUMMARY: Full-Time 8:30am-5:00pm Pay:
$40,000 (Salary) Activities Specialist is responsible for assessing
participants for social activity needs, connecting participants
with community social activities (ADH, Senior Center, gyms, etc.),
scheduling ADH attendance, scheduling IDT approved transportation
for community social activities and work with IDT to create
interventions to provide participants with services that help
ensure participant can continue to live independently in the
community. PRINCIPAL DUTIES AND RESPONSIBILITIES: The following
represents most of the duties performed by the position but is not
meant to be all-inclusive or prevent other duties from being
assigned when necessary: Participate as a member of the
interdisciplinary team (IDT) and in initial, semi-annual and
unscheduled assessments; attend morning updates and report changes
in participants’ baseline status to appropriate staff on a daily
basis. Involved in the development and implementation of QAPI
activities. Schedule ADH attendance. Schedule transportation for
ADH and IDT approved community transport. Ensure transportation
orders are active and correct in the system. Communicate with
transportation in the event a participant experiences an issue with
an activity related transport Monitor ADH attendance by
communicating with the ADH sites and reach out to
participants/family in the event of excessive cancelations Organize
bimonthly PAC meeting (contact participants/care givers for invite,
schedule rides as needed, conduct PAC meeting with QA and assist
with questions and concerns during this meeting.) Document on
participants who attend Complete timely documentation, including
semi-annual, unscheduled, and initial assessments; and other data
on participants as directed by the interdisciplinary team. Maintain
confidentiality of participant information. Be flexible in schedule
of hours worked. Requirements: CERTIFICATION, LICENSES,
REGISTRATIONS: Holds and maintains a current Driver’s license.
EDUCATION AND EXPERIENCE: At a minimum, GED or high school diploma;
prefer a certified nurse assistant (CNA) or bachelor's degree in
recreation, music, art, education, gerontology, or related field or
a current activity coordinator certification. Minimum of two (2)
years of demonstrated successful experience in
activities/therapeutic recreation related services with geriatric
population. Minimum of one (1) year of documented experience
working with a frail or elderly population. KNOWLEDGE, SKILLS, and
ABILITIES: Knowledge of activities/recreational therapy techniques
and practices of the elderly, including assessment, care plan
development, monitoring, and evaluation needs. Working knowledge of
physical, psychological, and behavioral needs of the elderly
population. Knowledge of the principles and techniques involved in
relating to frail elderly persons. Demonstrated creative abilities
to plan, develop, and implement varied activities to meet
diversified needs. Ability and experience working with volunteers.
Skill in communicating with participants, their families, and other
staff members; ability to establish a helping, trusting
relationship with participants. Ability to work effectively and
harmoniously with staff, elderly, public and governmental agencies,
providers of services, and peers. Well organized, dependable,
flexible, and resourceful. Effective oral and written communication
skills. Computer skills required. PHYSICAL REQUIREMENTS: Requires
constant hand grasp and finger dexterity; frequent sitting,
standing, walking and repetitive leg and arm movements, occasional
bending, reaching forward and overhead, squatting and kneeling.
Ability to communicate verbally, with an excellent comprehension of
the English language. Has access to and ability to drive one’s own
vehicle. WORK ENVIRONMENT: The main work environment is an office
setting of a non-profit organization providing a variety of
services to seniors and their families. The activity level is high
and the noise level is moderate. The work environment will include
a variety of clinician settings, day centers, hospitals, nursing
homes and individuals’ homes. Reasonable accommodations may be made
