Job Description Job Description Summary: The Registered Nurse
(RN) is responsible for assessing, planning, coordinating, and
implementing nursing care for patients receiving home health
services. This role includes care coordination, therapy referrals,
communication with physicians and care teams, patient and caregiver
education, and comprehensive documentation in the EMR. Shift: Days
| Monday–Friday | 8:00 AM – 5:00 PM Mileage Reimbursement: $0.70
per mile Travel Area: Skiatook and surrounding areas Sign-On Bonus:
$10,000 Relocation assistance available! Job Responsibilities:
Perform nursing skills according to assessed needs, physician
orders, and agency policies. Anticipate and deliver care in
alignment with home health standards and processes. Demonstrate
knowledge and proficiency in home health clinical skills. Educate
patients and families on disease management, emergency
preparedness, safety, and home care. Utilize correct medical
terminology and approved abbreviations. Skills & Experience
Required: Minimum of one (1) year of related nursing experience.
Strong clinical, interpersonal, and communication skills. Basic
computer proficiency. Reliable automobile for home visits.
Education Required: Graduate of an approved school of nursing with
a diploma or degree. Licensure & Certifications Required: Valid
State of Oklahoma or multi-state Registered Nurse (RN) license.
Valid driver’s license with an acceptable Motor Vehicle Report per
